Securing your appointment with Teresa
A non-transferable and non-refundable booking fee of £20 is required for each individual treatment to confirm your booking. This is payable via my link
You will receive a text message confirming your appointment shortly after booking, with full details.
A text message reminder will be sent to you 72 hours before your appointment. This will include a link to an online Medical Health Declaration Form and a Consent Form for Permanent Make-Up and/or aesthetics, as applicable.
Booking costs are collected with sum up, with the balance due during your appointment.
Appointments should be attended alone; children under the age of 16 cannot be accommodated at Mill Hill Beauty Bar.
New Clients
A text message will be sent to all new clients with a link to a Medical Health Declaration Form, Consent Form for Permanent Cosmetic Procedure, and COVID-19 Health Agreement. Please ensure that this information is completed and returned 72 hours before your consultation or appointment as applicable.
When booking your appointment, please provide your full postal address so that your home patch test kit can be mailed to you prior to your visit.
Please note that you must not have had any previous cosmetic tattooing or dermal filler in the same area that you are booking a treatment for. For example, if booking ombre brows, you must not have previously had microblading/ombre brows done by another practitioner. This applies also to dermal fillers.
If you do have previous work in the area, please contact Teresa Howard PMU and Aesthetics personally to discuss. If you book without first speaking with Teresa and it turns out that we are unable to work with you, your booking fee will be forfeited.
Permanent make-up is a two-part procedure; a second 'perfecting' session is required. It is your responsibility to book this appointment using the booking system, immediately following your treatment.
Follow-up sessions need to take place 4-6 weeks following your original appointment. After week 6, bookings will be charged as a Colour Boost session. Please see the treatment menu for the most up-to-date price information.
All Colour Boost sessions are priced for returning clients only.
Returning Clients
All returning clients will be required to fill out a Returning Client Medical Questionnaire and a Consent form for Permanent Cosmetic Procedure to confirm that their medical history has not changed.
Booking Conditions
To reschedule your appointment, you must give at least 7 days' notice prior to your treatment. If you do not offer 7 days' notice, your booking fee will be forfeited.
Please keep in mind that a rescheduled appointment cannot be scheduled on a Saturday. Saturday appointments will be automatically cancelled and rescheduled for the next available working day.
Regardless of the circumstances, you have the right to reschedule your appointment on a maximum of two occasions.
If you arrive more than 10 minutes late for your scheduled appointment, it may be cancelled, and your booking money will be forfeited.
Your booking fee will be forfeited if you fail to appear or forget your appointment without providing the required 7-day notice. To rebook a future appointment, a £50 non-refundable booking fee will be charged.
Top-up Appointments
A pre-treatment patch test is an important aspect of your treatment and must be completed at least 48 hours before your session.
If you cancel your 4-8 week top-up appointment without giving 7 days' notice, you will be charged a £50 cancellation fee. The cancellation charge must be paid before any future appointments may be made.
Teresa Howard PMU & Aesthetics prioritise our client time and as an owner-operator business, your understanding of this is appreciated. Therefore, we appreciate your support of our booking conditions and early notice of any requests to reschedule your treatment.
